Study from Jackson Physician Search Reveals Physician and Healthcare Administrator Discrepancies on Physician Retirement Plan Drivers and Timelines

ATLANTA and ORLANDO, Fla.  (March 20, 2023) — The full impact of the increase in physician retirements due to an aging physician population compounded by rising levels of burnout has yet to be felt by an industry already stretched thin.

Research results announced today from Jackson Physician Search – a leading firm in physician, physician leader and advanced practice provider recruitment – found that one in four physicians foresee burnout as the top driver in their retirement decisions. Yet, 50% of healthcare administrators think it’s age. The same study revealed that while economic concerns have delayed retirement for 38% of physicians, of the 24% who were planning to retire early due to COVID-19, 60% still plan do so. These are among several key findings from the study outlined in the new report “Preparing for the Wave of Physician Retirements: Survey Data Reinforces Need to Plan” available here.

“It all comes down to the question of when – not if – the healthcare industry will feel the full impact of forthcoming physician retirements. Through this study, we wanted to understand physicians’ views on the topic, as well as readiness in terms of planning by the organizations for which physicians work,” explains Tony Stajduhar, president of Jackson Physician Search. “What we learned is that most physicians – regardless of age – say they will retire as soon as they are financially able. This points to a critical need: healthcare organizations must plan and start recruiting and retaining physicians differently than how many do so today.”

Findings from the survey conducted in November/December 2022 indicate that healthcare administrators are not consistently aligned with physicians on how, or when, they plan to leave the workforce, nor do they appear to be as prepared as necessary. Some 69% of administrators say their organization doesn’t have a formal, written succession plan, yet 63% acknowledge that the typical amount of notice they receive from physicians is six months or less.

“When you consider that recruiting a culturally aligned physician in today’s competitive market often requires 6-12+ months and costs $250,000 or more – including sourcing, relocation and sign-on bonus – greater emphasis on initiating the retirement discussion, as well as accommodating physicians’ desire to slowly transition from full-time work to retirement, is key to keeping them in the workforce longer,” notes Stajduhar.

Responses to the survey were received from 387 physicians across a range of specialties and from 60 healthcare administrators throughout the country. In the report “Preparing for the Wave of Physician Retirements: Survey Data Reinforces Need to Plan” released this week, key takeaways include:

  • Administrators are not fully aligned with physicians on what constitutes the ideal amount of retirement notice. Physicians indicate six months is enough time; administrators prefer 1-3 years to allow sufficient time to recruit.
  • Only 12% of physicians intend to set a retirement date and fully retire, yet administrators believe that while physicians will slow down in their final years, they will then opt to leave medicine for good.
  • Nearly 60% of Gen X physicians say they plan to retire by age 60, and data shows they’re making a move in that direction with 12% of those aged 51-60 already working part-time.
  • When asked which options would cause physicians to consider delaying full retirement, 58% said part-time status, 52% said flexible schedules, and 42% said reducing or eliminating on-call requirements.

“While our research shows the desired retirement age is trending younger, it also provides valuable insight on what can be done to encourage physicians to stay in the workforce in some capacity,” adds Stajduhar. “Implementing full-scale retention and succession strategies are essential to lessening the impact of the coming wave of retirements.”

Additional research and insights found in the report include steps for creating a physician succession plan to help administrators anticipate and better prepare for physician retirements.

MGMA and Jackson Physician Search Release Findings on Physician Burnout, Engagement and Retention

ENGLEWOOD, Colo. and ATLANTA  (October 10, 2022) — New research from the Medical Group Management Association (MGMA) and Jackson Physician Search, a leading firm in physician recruitment, confirms that physician burnout and turnover rates remain dangerously high due to the rising toll of stress during the COVID-19 pandemic and resulting staffing shortages.

The new report – Back from Burnout: Confronting the Post-Pandemic Physician Turnover Crisis – reflects data from hundreds of administrators and physicians across the country who took part in the 2022 Physician Burnout, Engagement and Retention Survey, commissioned by Jackson Physician Search in partnership with MGMA and conducted in August 2022. The survey sought these industry leaders’ unique views on physician issues following the pandemic and staffing crisis in healthcare, and to understand how medical groups can improve physician recruitment, engagement and retention efforts through identifying root causes of burnout and steps to mitigate it.

Key Findings

After qualitative interviews with administrators and physicians, the report — a follow-up to a 2021 whitepaper — revealed several key themes:

  • Nearly two-thirds of physicians (65%) report they are experiencing burnout in 2022, up four percentage points from the 2021 study.
  • Of those experiencing burnout, more than one in three physicians (35%) said their levels of burnout significantly increased in 2022.
  • Administrators acknowledge worsening levels of burnout in physicians, but physicians often don’t perceive enough is being done to mitigate that burnout or engage them.
  • Administrators vary their approaches to retention and engagement, often with informal efforts rather than structured, strategic programs.
  • Perhaps most telling is that 51% of physicians have considered leaving their current employer for another, up from 46% last year.

In addition to these newly released survey findings, recent MGMA polling found that four in 10 medical practices (40%) had a physician resign or retire early in the past year due to burnout.

“While burnout is certainly not new, it’s disheartening that one in three physicians is reporting a significant increase in their level of burnout in 2022,” said Tony Stajduhar, president of Jackson Physician Search. “Administrators and physicians recognize that solving the burnout crisis is complicated as many of its drivers are rooted in the business of healthcare. However, burnout mitigation should include creative recruitment and staffing strategies that bring additional talent into the organization to help defuse the impact of stress and administrative overload, and that allow for the pursuit of physician work-life balance.”

What’s Being Done to Confront Burnout and Turnover

The new Back from Burnout: Confronting the Post-Pandemic Physician Turnover Crisis report points to the fact that most medical groups are discussing burnout and turnover but have yet to implement specific strategies to address these mission-critical issues. In fact, MGMA polling during the pandemic found that nearly 9 in 10 healthcare leaders said they did not have a formal plan or strategy to reduce physician burnout. This new report confirms that with just 19% reporting having a formal, written retention plan.

With one in two physicians seriously considering leaving their current employer for a new one and 36% considering early retirement, healthcare leaders must take heed. The report details that mitigating burnout and rising rates of physician turnover may best be achieved through streamlining clinical workflows, managing workload equity amid ongoing staffing shortages, improving two-way communication between physicians and administrators, and increasing psychological safety in the workplace to encourage quality, honest feedback.

Jackson Physician Search and LocumTenens.com Release New Study: Rural Physician Recruitment and Staffing

ATLANTA, (February 24, 2022) — A new survey from Jackson Physician Search, a permanent physician recruitment firm, and LocumTenens.com, a temporary physician staffing firm, provides fresh insights to help rural healthcare organizations make progress in resolving their chronic physician recruitment and staffing challenges. Takeaways from the study titled Rural Physician Recruitment and Staffing Survey Results suggest the market could be shifting, as 90 percent of surveyed physicians indicate they have some level of openness to considering rural practice.

“Our findings provide a recruitment roadmap for rural healthcare,” said Tony Stajduhar, president of Jackson Physician Search. “Considering the reality that 20 percent of the population lives in rural areas — yet only about 10 percent of physicians practice there — combined with COVID-prompted early retirements and job changes, these new insights suggest that rural healthcare organizations can more effectively market open positions by better understanding what physicians want and need.”

The recent survey of 1,311 physicians and 169 administrators, as well as 158 advanced practice providers, explored the dynamics of what it’s like to work in rural, suburban and urban healthcare environments, and how physicians are thinking about rural practice opportunities. Interestingly, physicians sometimes vary in their viewpoints based on their current practice location and stage of life.

In addition, administrators were asked about the likelihood that they might consider locum tenens as part of their staffing strategies to meet the immediate needs of their communities — whether that be for a temporary gap in coverage or to mitigate the negative impact on patients and organizational revenue during a lengthy vacancy.

The new report, Rural Physician Recruitment and Staffing Survey Results, details:

  • Physicians’ and advanced practice providers’ sentiments on what they want in a permanent rural opportunity and how these compare to what administrators think
  • Physicians’ and administrators’ views on rural locum tenens, as well as recommendations for its strategic use
  • Insights into rural physician retention, including what physicians value most when considering whether to stay
  • A snapshot of how COVID-19 is impacting physician retirements and rural recruitment

Physicians are clear about what they want; administrators can take heed

The study found that physicians who are currently practicing in urban and suburban locations are looking for higher compensation, bonuses and benefits (64 percent), more flexible hours (47 percent) and improved work-life balance (46 percent), and many are open to finding those characteristics in a rural opportunity. Administrators, on the other hand, ranked compensation as the fifth most important factor, indicating there’s a disconnect that must be overcome. Noted Stajduhar, “This suggests that if physicians are well paid and assured of better work-life balance, a rural location may be less of a negative factor than once believed.”

What can be learned about how cultural attributes affect the rural experience during recruitment and for retention purposes?

“We have long advocated that physician retention begins with recruitment,” explained Stajduhar. “And this survey confirms that finding the right cultural ‘fit’ is the first step. Understanding what physicians value most in a facility’s culture and the factors that will motivate them to stay can go a long way towards making a good match and keeping them long term.”

Administrators underestimate the importance of providing physician autonomy. Just 24 percent of those surveyed feel it’s an attribute of culture that most appeals to physicians. However, physicians (40 percent), regardless of practice location, rank it as the most important aspect of culture. And what will motivate physicians to stay? While physicians and administrators agree on the top factors influencing physician retention, physicians (55 percent) place greater value on higher compensation, bonuses and benefits than administrators (37 percent) believe.

Temporary staffing might make more economic sense than administrators think

The survey asked physicians who don’t already work in a rural setting if they might consider a locum tenens assignment to “try out” a rural location — and 72 percent of physicians responded favorably. The study results also revealed that nearly 1 in 10 of physicians who moved to a rural practice cited COVID-19 as the reason.

“Understanding what motivates both physicians and administrators today — versus what might have been important to them pre-COVID — is worth exploring,” said Chris Franklin, president of LocumTenens.com. “Traditionally, temporary assignments have had the most appeal for those just starting out or those on the cusp of retirement. This survey suggests that it’s possible physicians might want to shake things up mid-career.”

Administrators also may want to re-think locum tenens as another recruitment strategy. “Rural facilities may view locum tenens as a more costly short-term solution when facing a vacancy, but it is all about perspective,” added Franklin. “While recruiting a permanent physician to a rural facility is the goal, there are different approaches to get there. For instance, if a vacancy is costing lost revenue, decreased access to care and the risk of increased burnout on existing staff, facilities can leverage strategic recruiting to identify temporary locums candidates who, in many cases, may be open to permanent placement in the long run.”

Jackson Physician Search and MGMA Release New Study: Getting Ahead of Physician Turnover

SAN DIEGO (October 25, 2021) — It might appear that the business sector most impacted by the Great Resignation phenomenon of 2021 is healthcare, with a high percentage of nurses, physicians and other providers reporting to be burned out, dissatisfied and considering a career change. But is COVID the cause? A new survey conducted in June and July 2021 by Jackson Physician Search in partnership with Medical Group Management Association (MGMA) suggests that physician satisfaction with their current employer — reported at an average of 5.5 on a scale of 0 to 10 — is less about the impacts of the pandemic and more about disconnects that can be solved, at least in part, by better communication. This and other insights around recruitment, retention and succession planning are available in a new whitepaper, Getting Ahead of Physician Turnover in Medical Practices, released today at the MGMA Medical Practice Excellence Leaders Conference.

“Successful business decisions begin by recognizing threats and opportunities, and the pandemic exposed many for healthcare organizations, so we’ve seen a rapid and sustained increase in the number of physicians actively looking for new jobs,” said Tony Stajduhar, president of Jackson Physician Search. “But what we’re learning is that post-COVID business recovery actually requires making the experience of being a doctor better for physicians. Recognizing the all-consuming nature of practicing medicine and considering physicians as partners versus employees are where it starts.”

Sixty eight percent of administrators and 61% of physicians confirm that physicians are experiencing burnout. Of those, 62% of physicians reported burnout as being caused by their current employer (versus being caused by the experience of being a physician), while only 14% of administrators recognized their role in physician burnout. 

Physicians are clear: they want to be heard

Interestingly, only 25% of physicians attributed physician burnout to COVID-related stress, while 37% of administrators attributed it to COVID. That sense of disconnect between physicians and administrators is reflected elsewhere in the data, particularly around the perceived effectiveness of communication.

“Administrators point to many different conditions that would address physician burnout, like having better clinical support, boosting compensation and lowering administrative burdens,” said Dr. Halee Fischer-Wright, CEO of MGMA. “However, far and away, two-way communication with management and administrators is cited by physicians as the most important factor in keeping them satisfied in their current position.”

Succession planning is a critical need

Considering the chronic physician shortage, practices can’t afford not to plan for succession. However, according to the survey, 67% of administrators reported not having a succession plan, with only 16% answering in the affirmative. For those who do have a plan in place, existing succession plans focused on recruitment to replace retiring physicians and transitioning others to part-time roles.

“The takeaway for administrators is to step back from the day-to-day long enough to look at longer time horizons; consider the impacts of growth, retirement and other transitions; and put a plan in place that acknowledges the realities of their practices and practice requirements,” explained Stajduhar. “The whitepaper Getting Ahead of Physician Turnover in Medical Practices lays out a strategy for administrators to consider in building both a long- and short-term succession plan for physicians to minimize the impacts on patient care.”

Improving physician recruiting efforts presents a great opportunity

Once a succession plan has been developed, administrators will be better equipped to recruit physicians, another area in need of alignment based on the study’s results. Administrators rated their organizations’ physician recruitment strategies at 7.0, much higher than physicians rated efforts at 4.8, on a scale of 0 to 10.

“Right now, there is an opportunity for growth in how physicians are recruited,” said Dr. Fischer-Wright. “Typically, physician recruiting takes place over a lengthy time, requires a strong fit of both skill and culture, and is a significant financial investment. Creating a high performance recruiting program is a critical next step for administrators.”

Getting Ahead of Physician Turnover in Medical Practices is the result of a survey that was fielded in June and July 2021 by Jackson Physician Search in partnership with MGMA.
